On the day of your appointment or procedure, please bring the following:
We accept PPO insurance, Medicare, Workers’ Compensation and cash payments. For co-pays, we accept cash, credit and debit cards.
Insurance requires collection of a co-pay at the time of your appointment. If you cannot submit your co-pay prior to your appointment, we will honor your appointment, but there is a $25 fee for submitting your copay after your appointment.
